Enrollment Instructions
1. Fill out online application and provide documents
a. Health insurance card (if applicable).
b. Most recent individualized education plan (IEP).
c. Most recent evaluation team report (ETR) from school.
d. Evaluation report showing autism diagnosis from physician.
2. Schedule an intake assessment and initial clinical interview.
3. Verification of coverage.
4. Decide to receive services.
5. Finalize enrollment and provide remaining necessary documentation.
6. Meet with your BCBA to finalize the treatment plan.
7. Begin receiving services!!!
